CHANGE OF STATUS and LEAVE REQUEST FORM

I understand that in order for this change in status to be approved in good standing I must be current with all financial obligations to the JLA and A Christmas Affair. I understand that if I am resigning, transferring to another League, or changing to Non-Resident status, it is my responsibility to notify my Project Chair or Committee Chair of this change of status. I understand that the official effective date of change is as follows: the date the Placement Chair approves the resignation request, the date the Board approves the resignation request, the date the Membership Council approves the Non-Resident request, the date the transfer is submitted electronically to AJLI, or June 1st for all Sustainer and Associate Active requests. Finally, I am aware that certain conditions of membership in the JLA must be met in order for my request to be approved.
